Outrigger Collapse

I hear a bit about this rather alarming/embarassing/expensive occurrence. Does
anyone know how often this happens or has it only been once which is being
repeated?

Also, if it happens a lot does anyone know if anything is being done to find out
the cause and correct what must be the fault?
