Hi Jim, 

Sounds like you are getting close! The big reference is the FAA Advisory 
Circular
90-89A. Our EAA Flight Advisor gave us additional handouts and testing 
suggestions.
We used all of this to develop our plan and made a set of test cards
for each flight, one for the person on the ground and an abbreviated set for the
pilot. Unfortunately, we don't have them in electronic format, but if you'd
like, we can bring the hard copies we used to Rough River.
